Sign Up for the REALIZE mySUCCESS^® Program

Follow these steps to access REALIZE mySUCCESS^® presurgery features and tools now.

1. Go to www.REALIZEmySUCCESS.com.
2. Click the PATIENT LOGIN button.
3. Select your country of residence and click Register.
4. Complete your registration in 5 steps. (You'll need a valid e-mail address to register.)
   1. Read and accept the Terms of Use and Privacy Policy.
   2. Answer the questions about your surgeon, your surgery date, and the procedure type if you know it.
   3. Confirm your surgeon's information if you know it. If you can't find your surgeon's information, call 1-866-REALIZE (1-866-732-5493) for assistance. We encourage you to check the box indicating your permission to allow your surgeon to see your progress and step in when needed to keep you on track. You can change your preference anytime under myProfile.
   4. Enter the requested information, including your gender, date of birth, starting weight, and height. Click NEXT.
   5. Finalize your account information by entering your e-mail address and choosing a password. Select a security answer. Click FINISH.

Accessing the full set of REALIZE mySUCCESS^® features after surgery

After surgery, if you choose the REALIZE^® Solution^† for gastric banding, gastric bypass, or sleeve gastrectomy, you'll be able to explore many more features and resources. You'll be able to:

• Access all recipes
• Set up appointments and reminders

• Track your progress
